Every activities sponsored by CIC is carefully planned and adequatlely supervised by mature adults. However, even with best of planning and precaution, unforseen events can occur. The parents and guardians agree to assume and accept all risks and hazards including the possibilty of getting hurt while playing , or contacting inffectous diseases when children interact with each other. They also agree not to hold CIC and school staff/volunteers liable for any sickness or injury to the person or property undersigned They agree that they will not file legal action against CIC or the staff. The parents or guardians understand that they are signing for the minor(s) listed on this form and the signature is for both a medical and liability release.
